WellFort Community Health Services is seeking a compassionate, skilled, and collaborative Physician Assistant (PA) to join our team.

The Opportunity
The Physician Assistant provides comprehensive, community-oriented primary health care with an emphasis on accessibility, health promotion, illness prevention, and continuity of care.
Working within WellFort's interdisciplinary model, the Physician Assistant will provide clinical care to clients across diverse communities and populations. The PA will work both independently and collaboratively with physicians and other healthcare professionals, in accordance with applicable medical directives, organizational policies, and the direction of supervising physicians.
WellFort's multidisciplinary teams may include family physicians, nurse practitioners, registered nurses, medical office assistants, registered dietitians, social workers, physiotherapists, diabetes educators, chiropodists, and other health and community service professionals.

General Responsibilities
The Physician Assistant will:
- Provide comprehensive primary care within the scope of practice, organizational policies, and approved medical directives
- Conduct patient assessments, including comprehensive medical histories and physical examinations
- Assess, diagnose, and manage acute and chronic medical conditions in collaboration with supervising physicians
- Order and interpret appropriate diagnostic tests and investigations in accordance with applicable medical directives
- Develop and implement individualized patient care plans
- Provide follow-up care and support continuity of care
- Perform appropriate minor procedures within the PA's scope of practice and organizational medical directives
- Provide patient education related to preventive health, medications, chronic disease management, and healthy lifestyle practices
- Support health promotion, disease prevention, and population health initiatives
- Coordinate care with physicians, nurses, allied health professionals, community services, and other members of the healthcare team
- Identify barriers to care and support clients in accessing appropriate health and community resources
- Advocate with and/or on behalf of clients when appropriate
- Participate in interdisciplinary case discussions and team meetings
- Participate in quality improvement initiatives and organizational projects
- Apply evidence-based practice to improve client and system outcomes
- Maintain complete, accurate, and timely clinical documentation within the Electronic Health Record
- Participate in organizational initiatives that support WellFort's vision, mission, values, and commitment to health equity
Clinical Practice
The Physician Assistant will perform clinical duties under the direction of designated supervising physician(s) and within WellFort's written organizational medical directives.
Clinical responsibilities may include:
- Patient history and physical examination
- Assessment and management of acute and chronic illness
- Preventive health and screening
- Chronic disease management
- Medication management
- Ordering and reviewing laboratory investigations and diagnostic imaging
- Follow-up and monitoring of treatment plans
- Patient and family education
- Minor procedures, as appropriate
- Referral and care coordination
- Health promotion and disease prevention
- Documentation and communication of clinical findings and treatment plans
The PA will exercise appropriate clinical judgment while recognizing the importance of consultation and collaboration with supervising physicians and other members of the interdisciplinary team.
Interdisciplinary & Community-Based Care
WellFort's model recognizes that health extends beyond the clinical encounter. The Physician Assistant will work collaboratively with healthcare providers and community partners to address the broader needs of clients and communities.
The successful candidate will work cooperatively with members of the Primary Care and Allied Health teams, which may include:
- Family Physicians
- Nurse Practitioners
- Registered Nurses
- Medical Office Assistants
- Registered Dietitians
- Registered Social Workers
- Physiotherapists
- Diabetes Educators
- Chiropodists
- Other healthcare and community service providers

Advocacy & Community Development
The Physician Assistant will contribute to WellFort's broader community health mandate by:
- Identifying health needs and barriers experienced by clients and communities
- Supporting initiatives that improve access to primary care
- Participating in organizational committees and working groups, as appropriate
- Sharing observations regarding emerging health needs and service gaps
- Supporting health promotion and community development initiatives
- Participating in relevant Physician Assistant education, networking, and professional development opportunities
- Contributing to initiatives that advance health equity and reduce barriers to care
Quality Improvement & Evidence-Based Practice

Accountability
The Physician Assistant is accountable to designated supervising physician(s) for clinical care and practice within applicable medical directives.
The Physician Assistant is accountable to the appropriate WellFort Manager for the overall provision of service, operational responsibilities, and adherence to organizational policies and procedures.
Qualifications, Experience & Skills
- Graduate of an accredited Physician Assistant program
- Current (or planned for new graduates) certification with the Canadian Association of Physician Assistants (CAPA)
- Current (or planned for new graduates) regulation under the College of Physicians and Surgeons of Ontario (CPSO)
- New graduates are welcome to apply
- Experience in family medicine, primary care, community health, or a related setting is considered an asset
- Excellent clinical judgment and assessment skills
