In this position, you will be deployed to our partner site, Rexdale Medical Centre, while remaining an employee of WellFort Community Health Services.

General Responsibilities  

The Physician Assistant provides comprehensive, community oriented primary health care, which emphasizes accessibility, health promotion, illness prevention and continuity of care.  

Rexdale Medical Centre is a modern, well-equipped facility operating a busy, multidisciplinary family practice with 10 family physicians, 4 receptionists, 3 nurses, and a dietician.  The team provides compassionate, comprehensive primary care services to patients of all ages.  Rexdale Medical Centre is looking for a skilled and caring Physician Assistant to join its collaborative and supportive healthcare team. 

The Physician Assistant functions both independently and as a part of an interdisciplinary team through collaboration and medical directives. The Physician Assistant will support all quality improvement initiative and operate within the following frameworks; Open Focused Care (Advanced Access), Anti Oppression, Asset Based, Fluid and Mobile and Health Promotion.  

 Accountability   

  • The Physician Assistant is accountable to the supervising physician(s) at Rexdale Medical Centre for client care.  
  • The Physician Assistant is accountable the Manager, Integrated Care at WellFort for the overall provision of service.  

Specific Responsibilities  

  • Perform clinical duties within the parameters of WellFort’s written organizational Medical Directives and under the direct authority of the designated Supervising Physician(s) including;  
  • Conducting patient assessments, including history taking and physical exams 
  • Diagnosing and managing acute and chronic medical conditions 
  • Ordering and interpreting diagnostic tests and imaging 
  • Developing and implementing patient care plans in collaboration with supervising physicians 
  • Performing minor procedures (e.g., wound care, suturing, injections, blood collection as appropriate) 
  • Providing patient education on preventive health, medications, and lifestyle management 
  • Coordinating care with physicians, nurses, and allied health professionals 
  • Participating in quality improvement and team meetings 
  • Maintain complete and accurate records of her/his assessments and treatments within the Electronic Health Records system (PS Suite).  
  • Conduct interventions with and/or advocate on behalf of clients.  
  • Assess and improve quality of client/system outcomes through evidence-based practice . 
  • Work in cooperation with other members of the Primary Care Team including Family Physicians, Nurse Practitioners (NP), RN's, Medical Office Assistants, Registered Dieticians, Registered Social Workers, Physiotherapists, Diabetes Educators, Chiropodists.  

  Advocacy & Community Development:  

  • Assist in planning and programming of WellFort and Rexdale Medical Centre by participating on committees and by sharing information about perceived health needs and problems in the community, obtained through client encounters and other community contacts.  
  • Maintain contact with and participate in relevant physician assistant training networking opportunities.  

 

QUALIFICATION, EXPERIENCE & SKILLS 

  • New grads welcome 
  • Graduate of an accredited Physician Assistant program 
  • Current certification with the Canadian Association of Physician Assistants (CAPA) 
  • Current regulation under the College of Physicians and Surgeons of Ontario (CPSO) 
  • Experience in family medicine or primary care an asset 
  • Excellent clinical judgment, communication, and interpersonal skills 
  • Commitment to patient-centered, compassionate care 

 

Hiring Wage Range: $42.85-55.00 hourly (consideration of experience)
