The Site Team Lead is responsible for the day-to-day operational performance of the assigned clinical site, ensuring smooth and efficient clinic flow and use of resources in alignment with the goals of programs and WellFort’s values policies and procedures.

The Site Team Lead reports directly to the Manager, Primary Care, ensuring alignment with organizational priorities and clinical operating standards. It also maintains a dotted-line reporting relationship with additional Program Managers whose teams operate within the assigned site, supporting cross-program coordination, integrated care and consistent operational practices. The Site Team Lead collaborates closely with these Program Managers to ensure seamless workflow alignment across all site users.

As the on-site expert in WellFort’s standard operating procedures, the Site Team Lead ensures consistent adherence and actively promotes and upholds WellFort’s core values in all aspects of site operations, team interactions, and client interactions.


SPECIFIC RESPONSIBILITIES

• Maintain a consistent on‑site presence five days per week to provide real‑time leadership, supervision, mentorship, and operational support to team members, ensuring alignment with the programs strategic priorities.

• Liaise with and support facilities to ensure smooth and efficient site operations.

• Lead day‑to‑day clinic operations including scheduling, client flow, service volumes, and intervention timing, while escalating complex or systemic issues to the Manager, Primary Care, as required.

• Exercise delegated authority to make day‑to‑day operational decisions regarding scheduling, workflow adjustments, and resource allocation within approved limits.

• Consult with the Manager, Primary Care, on matters requiring higher‑level oversight such as HR concerns, budget approvals, risk issues, and strategic decisions.

• Support the Manager in planning and coordinating clinical services both within WellFort locations and in co‑located community settings.

• Consults with, and escalates issues to other program managers, as appropriate.

• Serves as primary on-site point of contact for staff, partners, and external stakeholders.

• Work with facilities to identify site-specific issues.

• . Assist in developing and refining protocols, tools, and procedures that support standardized, high‑quality care, in collaboration with the Manager and clinical leadership.

• Document and develop process flows, referral pathways, and other documentation that assists team members.

• Promote a positive, client‑centered culture and facilitate effective communication across on‑site teams.

• Address minor conflicts and support constructive resolution, escalating complex issues to the Manager as appropriate.

• Order supplies within predefined limits and approved budgets and tracks usage.

• Make operational decisions with clear escalation protocols.

• Disseminate information to frontline staff and coordinates responses.

• Collect and submit data to support performance indicators. Identify staff training needs and supports learning on the job.

• Monitor site performance metrics and quality indicators to provide recommendations to the Manager to enhance efficiency and service quality.

• Lead regular team meetings and huddles to review key performance indicators, operational updates, and expectations set by leadership.

• Facilitate effective communication across all teams on-site, ensuring alignment and timely information flow.

• Liaise with community partners and external service providers to support integrated care, in alignment with the Manager’s strategic partnership goals.

• Document problems identified using a specified framework.

• Provides input to annual performance development plans of team members.

• Demonstrate effective time management, planning and organization of day’s work activities and of team members.

• Participate in staff planning and leads orientation of new staff and students.

• Responsible for first line risk mitigation strategies in collaboration with the Manager’s ultimate accountability.

• Identify operational challenges, proposes solutions, and escalates issues proactively to leadership when necessary.

• Uphold a positive, client-centred culture, addressing concerns and fostering strong team engagement.

• Participate in site-level audits, continuous quality improvement efforts.

• Sit on the Joint Occupational Health and Safety Committee.

• Support the building of an integrated team across WellFort CHS.

• Assist staff with supporting data-driven practices to ensure accurate data collection/documentation of activities and documentation of services.


Health Promotion and Education

• Identify trends or issues related to the health status of priority groups and uses the information in planning and advocacy work.

• Promote and increase awareness of health promotion activities with clients including immunizations, flu shots, smoking cessation, risk assessment, lifestyle modifications.


Advocacy, Community Development and Partnerships

• Advocate for and with clients to reduce barriers to accessing health care and other services in the health centre and the broader community.

• Conduct administrative tasks associated with referred services for non-insured clients including but not limited to, specialist offices, hospitals, diagnostic imaging, labs etc., and assist Manager of Primary Care with invoicing process.

• Work with clients, especially those who traditionally experience barriers to access to ensure that they are treated with fairness and dignity.

• Participate in the development and support of public policies and strategies that have a positive influence on the determinants of health.

• Participate in community projects or WellFort activities to provide education, information or to develop collaborative strategies.

• In collaboration with the Manager and Executive Leadership, ensure collaborative partnerships with key stakeholders of the program.

• Identifies gaps in service and makes recommendations to reduce these gaps

Administration

• Maintain clear, accurate, and timely written electronic documentation including EMR, requisitions, reports, encounters, and other information as required by the health centre, MOH, OH, other regulatory bodies.

• Contribute to policy/protocol development, in relation to health care service delivery.

• Maintain an up-to-date knowledge of government guidelines, acts and legislation affecting client care.

• Share responsibility for the ordering, maintenance, and proper handling of medications, vaccines, supplies, and equipment.

• Make purchases within allocated budget lines and follows protocol for ordering, tracking, and paying for supplies within financial policies

• Responsible for monitoring and reporting of all key data elements as per funder requirements.

• Provide back up to the Manager as required.

QUALIFICATIONS

• Three years’ experience within a community health setting.

• Degree in a related field from a recognized community college or university is considered an asset.

• An understanding of, and critical analysis of the determinants of health and social factors, such as power dynamics that affect individual, organizational and community health.

• Strong communication and organizational skills to work with Management and front-line healthcare staff to identify and prioritize business needs.

• Excellent written communication and oral skills.

• Excellent analytical and problem-solving skills.

• Demonstrated ability to develop and maintain collaborative working relationships, build credibility, consult, build consensus, coach, and resolve conflicts.

• Experience working with confidential and time sensitive documents and information.

• Excellent interpersonal skills and the ability to multitask in a fast-paced environment.

• Demonstrated ability to work independently and interdependently in an interdisciplinary team environment in which the skills and responsibilities of each team member are recognized and respected.

• Demonstrated proficiency in use of computers and various computer software applications, such as Electronic Medical Record (EMR).

• Registration with a Regulatory College in Ontario and in good standing an asset.

• Team Lead, Project Lead or supervisory experience an asset.

• Coaching, mentoring and conflict resolution skills an asset.

• Experience working with communities and individuals who face barriers accessing tradition health care institutions such as poverty, discrimination, mental health, and substance use issues an asset.

• Experience working in a value based non-profit or social services organization an asset.

• Ability to speak languages relevant to the residents of the organization’s community an asset.
