Position Summary

The Nurse Practitioner (NP) will integrate advanced knowledge of nursing practice and theory, comprehensive health assessment and diagnosis, therapeutic management, health promotion, disease prevention, and chronic disease management to provide comprehensive primary health care across the lifespan, including prenatal, pediatric, adolescent, adult, and older adult populations.

The NP will work both independently and collaboratively with individuals, families, caregivers, interdisciplinary team members, and other healthcare providers to deliver high-quality, evidence-informed clinical care and outreach services.

Care will be family-centred, trauma-informed, culturally responsive, and developmentally appropriate, addressing physical, mental, and social health needs, as well as the broader social determinants of health.

While the proportion of time allocated to specific functions may vary according to service needs, the advanced nursing role encompasses a combination of direct primary care across the lifespan, health promotion, patient and caregiver education, advocacy, community development, program development, and evaluation, with an emphasis on improving health outcomes for individuals and families within the community.


Accountability

The NP will practice within the full scope of practice as outlined in, but not limited to, the following documents:

  • College of Nurses of Ontario Nurse Practitioner Practice Standard
  • Nursing Act (1991) and Regulated Health Professions Act (1991)

The NP will maintain privacy and confidentiality in accordance with the Canadian Nurses Association Code of Ethics and adhere to the policies and procedures of WellFort.

The NP will report to an assigned Manager.


Specific Responsibilities

Health Assessment and Diagnosis
  • Perform focused and comprehensive health assessments, health histories, and physical examinations based on client needs across the lifespan (e.g., prenatal, postpartum, infants, children, adolescents, adults, and older adults).
  • Utilize strong clinical reasoning skills to synthesize assessment findings and evidence-informed practice to formulate differential diagnoses and communicate findings clearly to clients and families.
  • Consult and collaborate with other health professionals as required in establishing diagnoses and plans of care.
  • Order and interpret screening tests, diagnostic procedures, and other investigations within the CNO scope of practice and ensure timely follow-up.


Therapeutic Management
  • Collaborate with clients and families and consult with other health care providers to establish priorities and coordinate comprehensive plans of care.
  • Select appropriate non-pharmacological and pharmacological interventions, health counselling, and prevention strategies to restore, maintain, and promote optimal health.
  • Provide ongoing management of acute and chronic conditions using a client-centred, self-management approach, with regular evaluation and modification of care plans.
  • Support clients experiencing mental health concerns, substance use issues, emotional challenges, or complex social needs, and facilitate referrals to internal or external services as appropriate.
  • Participate in the development, implementation, monitoring, and evaluation of treatment, counselling, and health promotion services for individuals, families, and the broader community.
  • Dispense medications to clients without access to medications as delegated by a health centre physician and according to clinical protocols.
  • Provide services at outreach locations and maintain required communication with physicians or other team members through consultation.


Health Promotion & Education
  • Identify trends or issues affecting the health of priority populations and use this information to inform planning and advocacy efforts.
  • Participate in the planning, development, implementation, and evaluation of health promotion, education, and support programs relevant to the diverse communities served by WellFort.
  • Provide health information and counselling to individuals, families, and groups through discussions, workshops, presentations, written materials, and other educational initiatives.
  • Act as a clinical resource to staff and community partners regarding chronic disease prevention and management across the lifespan.
  • Promote preventive health measures including immunizations, cancer screening, sexual health, smoking cessation, risk reduction, lifestyle modifications, and readiness-for-change assessments.


Advocacy & Community Development
  • Advocate for and with clients and families to reduce barriers to accessing health care and community services.
  • Work with individuals who experience systemic barriers to care (e.g., poverty, discrimination, housing instability, mental health and substance use challenges) to ensure equitable, respectful, and dignified service.
  • Contribute to public policies and community strategies that positively influence the determinants of health.
  • Engage in community partnerships and initiatives to strengthen community capacity and promote healthy public policy.


Administration
  • Maintain clear, accurate, and timely electronic documentation (EMR), requisitions, reports, and other records in accordance with health centre, MOH, CNO, and regulatory requirements.
  • Contribute to policy and protocol development related to primary health care service delivery and professional practice.
  • Maintain current knowledge of relevant legislation, clinical guidelines, and evidence-based practices.
  • Support students and interdisciplinary learners, with emphasis on primary health care, determinants of health, and collaborative practice.
  • Share responsibility for the ordering, storage, and handling of medications, vaccines, supplies, and equipment.


Professional Development
  • Maintain professional competence in accordance with CNO Practice Standards.
  • Maintain registration in good standing with the CNO.
  • Maintain membership in relevant professional organizations (e.g., RNAO, NPAO).
  • Participate in Quality Improvement initiatives, chart audits, peer review, and clinical supervision as required.
  • Engage in ongoing professional development, continuing education, reflective practice, and lifelong learning.


Research & Teaching
  • Participate in research initiatives as opportunities arise.
  • Support clinical placements and academic partnerships, including supervision of nursing students, medical learners, and family practice residents.


Other Responsibilities
  • Provide care at any office or outreach location as program needs dictate.
  • Maintain access to reliable transportation to support responsive community-based care delivery.
  • Participate actively in interdisciplinary teams, staff meetings, committees, and centre-wide initiatives.
  • Promote inclusion, equity, and discrimination-free practice consistent with WellFort’s mission and values.
  • Maintain confidentiality and uphold the professional reputation of WellFort.
  • Develop and maintain a flexible schedule to meet the needs of clients and the community.


Qualifications

  • Undergraduate degree in Nursing required; Master of Science in Nursing (MScN) preferred.
  • Current registration with the CNO as a Registered Nurse Practitioner.
  • Current membership with RNAO or NPAO and evidence of liability insurance required.
  • COUPN Primary Health Care Nurse Practitioner Certificate is an asset.
  • Experience in diverse primary care settings across the lifespan is preferred.
  • Completion of a CNO-approved prescribing course for narcotics and controlled substances is an asset.
  • Demonstrated experience working with underserved and marginalized populations.
  • Strong understanding of the social determinants of health and structural inequities impacting individuals and families.
  • Excellent communication, documentation, and interpersonal skills.
  • Experience with EMR systems and proficiency with computer applications.
  • Ability to work both independently and collaboratively within an interdisciplinary team.
  • Additional languages relevant to the community served are considered an asset.
