Position: System Navigator
Location: Outreach to the South Surrey/White Rock community
Hours: 35 hours/week, mostly weekdays with one evening/week
We are looking for a System Navigator to join our Sources Housing Support Network team! This individual must be authentic, compassionate, and knowledgeable about the social services system. They must have a strong understanding and full acceptance of harm reduction philosophy.

Description of Position:
Reporting to the Manager of Peer Support Services, the System Navigator is a permanent employee of Sources Community Resources. This position will work as a team of two along with a Peer Navigator in an outreach capacity.
General Responsibilities:
- Outreach to various areas of the South Surrey/White Rock community to locate people who are experiencing multiple barriers to wellness, such as substance use, mental health challenges and homelessness
- Engage with clients in a trauma-informed manner to support them in developing physical, social, emotional and life skills
- Share lived-experience perspectives to build rapport and trust and modelling harm reduction practices, self-determination and wellness strategies
- Support clients in identifying personal goals for increased well-being
- Assist clients to find information about and gain access to local resources; supports clients to navigate and overcome barriers to receiving care; transports clients to resources and services while ensuring the security of unhoused client’s belongings
- Work collaboratively with clients as they set and work on their self-identified goals; support clients to break down goals into manageable steps, and assist them to achieve graduated success in all areas of their lives
- Provide opportunities for clients to increase their engagement in community integration activities; support clients to develop meaningful connections with a wide range of local resources and supports
- Responsible for case management, case note recording and monitoring goal attainment
Program Specific Responsibilities:
- Provide comprehensive case management to support achievement of client-identified goals
- Outreach along with Peer Navigator to foster relationships with vulnerable individuals
- Connect with other Peer Support Network drop-in services and collaborate with other service providers
- Transport clients to access health care and social services including but not limited to income assistance, substance use services, mental health supports, OAT
- Support clients to secure resources including but not limited to, shelter or sustained housing, identification, cell phone, food security
- Respond to urgent client needs as they arise
Qualifications:
- Experience navigating the systems of health care, mental health and substance use services and social services
- Lived experience of mental health, substance use, or housing insecurity will be considered as an asset in this role
- Diploma in a social-service related field plus experience related to supporting vulnerable populations
- Two years of recent related work experience with an emphasis on substance use and/or support to unhoused individuals
- Thorough knowledge of harm reduction and trauma-informed practice
- Proficiency in Motivational Interviewing or other engagement strategies are an asset
- Effectively utilize Office suite including Excel, Word, Outlook and central data systems
- Knowledge of community resources and programs
- Ability to analyze and problem solve
- Strong organization and prioritization skills
- Ability to engage with diverse clients who have a wide range of needs, abilities, strengths and challenges
- Ability to establish and maintain rapport and appropriate boundaries with clients and their families
- Ability to model, instruct and demonstrate life skills
- Effectively communicate professionally, verbally and written in English
- Valid Class 5 BC Driver’s License is highly preferable
- Standard First Aid and CPR (Training may be accessed through Sources)
- Non-violent Crisis Intervention Training (Training may be accessed through Sources)
- Criminal Records check through the Ministry of Public Safety and Solicitor General Criminal Records Review Program Valid Class 5 BC Driver's License and driver's abstract
