Position Description

Cubic serves as an objective, independent clinical resource for our plan sponsor clients and their members through solutions that have been implemented to optimize health benefits and ensure their long-term sustainability. This position builds on the successes that have been realized through the implementation of the Lead Pharmacist role at the Ontario Teachers Insurance Plan (OTIP), providing clinical consultation services to members and ensuring that they are receiving optimal care. The goal is to continue to positively impact disability management through pharmacist-driven early clinical intervention as well as assessment of unresolved Long-Term Disability (LTD) cases. The Pharmacist, Clinical & Early Intervention Services will collaborate with the Disability Management team at OTIP by acting as a clinical resource, and an integral part of their overall health care team, to ensure members are receiving optimal pharmacological and non-pharmacological treatments in a timely manner, with the goal of optimizing long-term health outcomes.

Primary Accountabilities

· Acting as a clinical and drug therapy resource for plan members off on leave or approved on LTD, as well as the Group Life and Disability Claims team

· Gathering all pertinent clinical information, conducting comprehensive patient reviews and pharmacotherapy assessments to resolve drug therapy problems and identify treatment gaps, ensuring optimal patient health outcomes and facilitating timely return to work where possible.

· Identifying gaps in cases that may have led to suboptimal outcomes to date and documenting recommendations for next steps.

· Conducting regular follow up assessments with patients to assess medication effectiveness, safety and compliance, and answering any medication and health-related questions and concerns.

· Liaising with Disability Analysts, Rehabilitation and Early Intervention Consultants, physicians, pharmacists and other healthcare professionals, acting as a subject matter expert and ensuring members have access to the right resources in collaboration with the broader disability team.

· Synthesizing and communicating scientific information at a peer level with physicians/pharmacists and communicating relevant information in lay terms to plan members.

· Developing and maintaining clinical resources and criteria that Disability Analysts, and Rehabilitation and Early Intervention Consultants can leverage to triage and refer more complex pharmacotherapy cases for pharmacy review.

· Developing processes and clinical workflows that can be used moving forward to ensure optimal outcomes.

· Working with OTIP and Cubic to identify data collection and integration opportunities that can be used to determine leading indicators for disability and proactively identify members at high-risk.

· Working with OTIP and Cubic to establish data integration processes that safeguard member privacy while enabling early intervention opportunities.

  • Ability to learn and capacity for scientific/therapeutic concepts and knowledge
  • Self-directed with an ability to work within a multi-disciplinary team
  • Comprehensive skills in MS Office including Excel and PowerPoint


Education & Experience

  • Pharmacy degree (B.Sc.Pharm or PharmD) with active Part A license
  • Licensure – Registered Pharmacist with the Ontario College of Pharmacists
  • 3+ years of experience as a licensed pharmacist, preferably in a hospital setting
  • Experience in psychiatry and mental health also an asset
  • Bilingual (in French and English) also an asset
