POSITION PROFILE

The Manager, Clinical Practice, in collaboration with the Area Manager, provides leadership and clinical oversight for Nursing and Personal Support Services across the Champlain region to ensure high-quality, evidence-based, and client-centred care. The Manager, Clinical Practice upholds best-practice guidelines, regulatory standards, and contractual requirements while managing daily clinical operations. Responsibilities include supervising staff, supporting clinical decision-making, driving quality improvement initiatives, and overseeing documentation and compliance. The position also leads skill development through coaching, education, and mentorship to strengthen clinical competencies across the team. As the Manager, Clinical Practice, the incumbent monitors key operational metrics and plays a vital role in promoting safe, effective service delivery and continuous professional growth.

ROLES & RESPONSIBILITIES

Clinical Leadership & Best Practice Oversight

  • Provide clinical leadership, guidance, and support to Nursing and Personal Support Services teams.
  • Ensure care is delivered in alignment with evidence-based best practices, Ontario Health atHome standards, CNO requirements, accreditation expectations, and all organizational/regulatory policies.
  • Act as the clinical escalation point for complex client situations requiring advanced assessment or intervention.
  • Promote a culture of safe, client-centred care in all service areas.
  • Support Care Managers and field staff in ensuring care plans are individualized and reflective of client needs and goals.
  • Lead the development and implementation of clinical policies and procedures.

Quality Assurance, Risk Management & Compliance

  • Conduct chart audits, documentation reviews, and client file assessments to ensure accuracy, completeness, and compliance.
  • Investigate incidents, support root-cause analysis, and implement corrective actions to reduce clinical risk.
  • Lead quality improvement initiatives that strengthen care delivery, client safety, and service consistency.
  • Participate in accreditation and compliance readiness activities.
  • Maintain clear communication with staff regarding policy updates, practice expectations, and quality initiatives.

Staff Supervision, Coaching & Skill Development

  • Provide ongoing mentorship and clinical support to nursing and PSW staff, fostering excellence and professional accountability.
  • In collaboration with the Area Manager, oversee staff performance through coaching, evaluations, and corrective action when needed.
  • Collaboratively manage orientation, competency validation, and continuing education for all field staff.
  • Identify skill gaps and develop targeted training or development plans to enhance clinical competencies.
  • Lead skill-development initiatives focused on assessment, documentation, client safety, and best-practice standards.
  • Ensure staff maintain required certifications and remain up-to-date with evolving clinical guidelines.

Clinical Operations & Coordination

  • Oversee daily clinical operations for nursing and PSW programs to ensure efficient workflow and alignment with service demands.
  • Monitor and respond to operational and clinical metrics to meet contractual targets and performance indicators.
  • Works closely with the Leadership and Client Services Teams to support service excellence.
  • Participate in interdisciplinary meetings, care conferences, and case reviews as required.
  • Represent the organization with external partners, including Ontario Health at Home and community agencies.
  • Support resolution of clinical concerns, complaints, and service recovery in collaboration with clients, families, and staff.
  • Support implementation of new programs, technologies, and workflow improvements.
  • Contributes to cost-effective service delivery practices.

Committees, Teams & Leadership Activities

  • Leads and oversees the Professional Practice Review Committee and specialty clinical teams.
  • Arrange and facilitate regular team meetings to support communication, collaboration, and continuous improvement.
  • Participate in the Manager On-Call rotation and provide after-hours clinical leadership.

QUALIFICATIONS

Registered Nurse in good standing with College of Nurses of Ontario (CNO)

Minimum 5 years clinical nursing experience in home and community care

Prior leadership, supervisory, or mentoring experience required (e.g., team lead, educator, supervisor, or manager role).

Strong understanding of CNO standards, home care regulations, and OHaH contractual requirements.

Excellent clinical judgment and ability to support complex client situations.

Exceptional communication, leadership, and interpersonal skills.

Ability to manage competing priorities, deadlines, and rapidly changing service demands.

Proficient in electronic documentation systems and Microsoft Office Suite.

Ability to travel within the service region as required.
