Job Description
As a Community Home Health Registered Dietician, you will play a vital role in supporting clients to achieve their nutritional goals and improve their overall health. You’ll work closely with patients in their homes, assessing dietary needs, developing personalized nutrition plans, and providing education and support. Your expertise will contribute to enhancing their quality of life and promoting overall well-being.
Responsibilities
- Conduct comprehensive nutritional assessments of clients.
- Develop individualized nutrition care plans based on assessment findings.
- Provide evidence-based dietary counseling and education to clients and their families.
- Monitor clients’ progress and adjust nutrition plans as needed.
- Collaborate with other healthcare professionals, caregivers, and family members to ensure holistic care.
- Advocate for clients’ nutritional needs within the healthcare system and community.
- Maintain accurate documentation of assessments, interventions, and outcomes.
Qualifications
- Bachelor’s or Master’s degree in Nutrition or Dietetics.
- Current registration with the College of Dietitians of Ontario.
- Minimum of 12 months experience in home health or community-based settings preferred.
- Strong communication and interpersonal skills.
- Compassionate, client-centered approach.
