Job Description

As a community Social Worker, you will play a vital role in supporting clients and their families as they navigate health, social, and emotional challenges. You’ll work closely with patients in their homes, assessing needs, developing care plans, and connecting clients to community resources. Your expertise will contribute to enhancing their quality of life and promoting overall well-being.

Responsibilities

• Conduct comprehensive psychosocial assessments of clients and families.
• Develop individualized care plans based on assessment findings.
• Provide counseling, crisis intervention, and emotional support to clients and families.
• Connect clients with appropriate community resources, services, and supports.
• Advocate for clients’ needs within the healthcare system and community.
• Collaborate with other healthcare professionals, caregivers, and family members to ensure holistic care.
• Maintain accurate documentation of assessments, interventions, and outcomes.

Qualifications

• Bachelor’s or Master’s degree in Social Work.
• Current registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
• Minimum of 12 months experience in home health or community-based settings.
• Strong communication and interpersonal skills.
• Compassionate, client-centered approach.
