Independent contractor (Part time; Full-time)

As a Physiotherapist working in the community, you will play a vital role in empowering clients to achieve their highest level of functional independence. This role will focus on supporting patients with complex needs by conducting in-home assessments, developing individualized treatment plans, and delivering therapeutic intervention. Your expertise will contribute to enhancing their quality of life and promoting overall well-being

Responsibilities:

• Provide community-based in-home physiotherapy services to clients referred to Access Healthcare Services
• Conduct comprehensive assessments of clients’ functional abilities and limitations.
• Assess clients’ needs, develop individualized treatment plans, and deliver evidence-based interventions.
• Collaborate with interdisciplinary teams to enhance clients’ overall well-being.
• Educate clients and their families on at-home exercise routines and preventive measures.
• Maintain accurate documentation of assessments, progress, and outcomes.
• Promote health and wellness within the community.

Qualifications:

• Bachelor’s or Master’s degree in Physiotherapy
• Current licensure as a Physiotherapist in Ontario
• Minimum of 12 months experience in home health or community-based settings preferred
• Strong clinical skills in assessment, treatment, and care planning.
• Excellent communication and interpersonal skills.
• Compassionate, patient-centered approach.
• Passion for improving clients’ quality of life.
