Independent contractor (Part time; Full-time)

As a Physiotherapist working in the community, you will play a vital role in empowering clients to achieve their highest level of functional independence. This role will focus on supporting patients with complex needs by conducting in-home assessments, developing individualized treatment plans, and delivering therapeutic intervention. Your expertise will contribute to enhancing their quality of life and promoting overall well-being
Responsibilities:
- Provide community-based in-home physiotherapy services to clients referred to Access Healthcare Services
- Conduct comprehensive assessments of clients’ functional abilities and limitations.
- Assess clients’ needs, develop individualized treatment plans, and deliver evidence-based interventions.
- Collaborate with interdisciplinary teams to enhance clients’ overall well-being.
- Educate clients and their families on at-home exercise routines and preventive measures.
- Maintain accurate documentation of assessments, progress, and outcomes.
- Promote health and wellness within the community.
Qualifications:
- Bachelor’s or Master’s degree in Physiotherapy
- Current licensure as a Physiotherapist in Ontario
- Minimum of 12 months experience in home health or community-based settings preferred
- Strong clinical skills in assessment, treatment, and care planning.
- Excellent communication and interpersonal skills.
- Compassionate, patient-centered approach.
- Passion for improving clients’ quality of life.
